WHAT IS DOUBLE EYELID SURGERY

Not bigger eyes finding the eyes that suit you

Double eyelid surgery is a surgery that creates an eyelid crease matched to your eye size and shape, opening strength, skin and the balance of the whole face. We look at all of these before choosing a method and shape.

Lines come in three kinds: in-fold, out-fold, in-out-fold. We listen to the impression you want, then suggest only what suits your eyelids.

THREE METHODS

Three methods, chosen by eyelid condition

Eyelid thickness · amount of fat · likelihood of loosening, decided together at consultation.

  • Non-incision (buried suture)

Suture fixation, no incision · thin eyelids · faster recovery

  • Incision

Loose skin and fat tidied · puffy eyelids · a line that rarely loosens

  • Partial incision

2–3mm opening · firmer than buried · less burden than full incision

PROCESS

Double eyelid surgery process

Every step is performed by the director.

  1. Line design

The height and shape of the line are set from the impression you want and your eyelid thickness and fat.

Double eyelid surgery process 01 — Line design
  2. Method · approach

Buried, incision or partial incision, whichever suits your eyelids, reaches the line position.

Double eyelid surgery process 02 — Method · approach
  3. Tissue tidying · fixing the line

Only the necessary fat and muscle are tidied, and the eye-opening muscle is connected to the skin to fix the line.

  4. Closure · follow-up

Closed with care, home the same day. Sutures come out after about 5 days, with 1–2 visits.

RECOMMENDED

If this sounds like you, double eyelid surgery comes first

  • A natural line on thin eyelids
  • A fold on one side only, or two sides that differ
  • Puffy eyelids that look swollen
  • A faint line that sometimes loosens
  • A loosened buried-suture line that needs firm fixing

WHAT IS PTOSIS CORRECTION

Not a surgery that makes a line one that restores the strength to open the eyes

Ptosis surgery is a surgery that adjusts the strength of the eye-opening muscle (the levator) to correct ptosis, where the eyelid covers the pupil. When that muscle is weak the eyes look sleepy, and the forehead does the opening.

It restores the strength to open the eyes, not the line, so the pupil shows clearly.

SYMPTOMS

Left for long, ptosis can lead to these changes

  • 01

    Forehead lines

Opening the eyes with the forehead deepens the lines

  • 02

    Uneven eyes

As the strength gap grows, the eyes look different in size

WHAT IS EPICANTHOPLASTY

Not enlarging the eyes releasing what feels closed

Epicanthoplasty is a surgery that opens the corners of the eyes to adjust their width and shape. Of the four directions (front, back, up, down), we open only the one that makes the eyes feel closed, and only as much as needed.

The epicanthal fold, eye width and outer-corner angle set the direction and amount, within the harmony of the whole face.

Canthoplasty surgery process 01 — Design · incision
  2. Fixing to the periosteum

The tip of the outer canthus is fixed firmly to the periosteum. This is the key step that keeps the corner from re-adhering.

WHAT IS EYE REVISION

In a second surgery, half is decided by assessment

Revision eyelid surgery is a surgery that re-addresses the problems of a previous double eyelid surgery: a line too low or too high, a loosened fold, scars, asymmetry, lingering swelling.

Six months to a year after the first surgery, once scars and swelling have settled, we begin with the assessment before surgery.

Eye revision surgery model, front view with even crease lines

EVALUATION

Before revision, we look at three things

These three decide the method and the limits. We tell you the possible range at consultation first.

  • Remaining skin margin

Range of line adjustment · whether drooping skin can be tidied

  • State of the scar tissue

Whether it can be tidied · refining an uneven line

  • Eye-opening strength

Whether ptosis is present · whether to combine ptosis surgery

CASES

The approach differs by type of problem

Six types we often meet at consultation. The actual method is decided after seeing the eyelids.

01

Line too low

Redesigned to the height you want, drooping skin tidied

02

Fold has come undone

The line is redesigned, fixed by incision if loosening worries you

03

Severe scarring

Rearranged into an even line that does not stand out, rather than erased

04

Swelling that lingers

The old line is released, scar, fat and muscle tidied, then a new line made

05

The two sides differ

Both sides are re-operated together rather than matching one side

06

Line too high

The high fixation is released and fixed lower, with fat grafting if needed

PROCESS

Revision eyelid surgery step by step

Every step is performed by the director.

  1. Cause analysis · planning

The cause of the previous problem is analysed, and after examination the method suited to you is decided.

  2. Design · releasing the old line

The new line is designed, and the tissue of the existing line is released.

Revision eyelid surgery step by step 02 — Design · releasing the old line
  3. Correcting the line · tidying scars

The misplaced line is released and scar tissue tidied. If needed, fat grafting above the eye creates the new line.

  4. Closure · suture removal

Closed with care, home the same day. Sutures come out after 5–7 days; swelling usually settles around a week.

RECOMMENDED

If this sounds like you, revision eyelid surgery comes first

  • A line too low or too high
  • A fold that has come undone or faded
  • An uneven double eyelid scar
  • Lingering swelling, thick-looking eyelids
  • Two lines that differ in height or shape

WHAT IS LOWER FAT REPOSITIONING

Removing fat is not the only way to clear the shadow

Under-eye fat repositioning is a surgery that moves bulging under-eye fat into the hollow tear trough through the conjunctiva inside the eye, to clear the shadow. As the tissue holding the fat thins, the fat bulges forward and the tear trough hollows, casting a shadow.

Sagging fat is removed; a bulge above a hollow is moved by repositioning. Both go through the conjunctiva inside the eye.

Not size, but fit is the standard.

We look for the line that looks as though those eyes were always that way. How comfortably they open matters more than how wide they open.

FAQ

Frequently asked questions

QBuried suture or incision, which is better?
Double Eyelid Surgery
There is no set answer. Thin eyelids with little fat get a natural line with the buried method; loose skin or puffy eyelids suit incision, and partial incision sits in between. We look at your eyelids at consultation and decide together.
QWill there be a visible mark?
Double Eyelid Surgery
The buried method has no incision, so it leaves almost no mark. Incision and partial incision lines are designed to sit inside the fold, hard to see with the eyes open. Scarring varies with skin condition and the individual.
QHow long is recovery?
Double Eyelid Surgery
Sutures come out after about 5 days, and light daily activities are possible around days 5–7. How long swelling and the line take to settle varies from person to person.
Ptosis Surgery
Sutures come out after about 5 days, and light daily activities are possible around days 5–7. How long the eyes take to settle varies from person to person.
Epicanthoplasty
Surgery takes about 30 minutes and you go home the same day. Light daily activities from days 1–2; sutures come out after 5–7 days, with 2–3 follow-up visits. This varies from person to person.
Under-Eye Fat Repositioning
Surgery takes about 30 minutes and you go home the same day. Light daily activities around days 5–7, with 1–2 follow-up visits. Swelling and bruising vary from person to person.
QCan the fold come undone?
Double Eyelid Surgery
The buried method fixes the line with sutures, so it can loosen over time. If that worries you, we suggest partial incision or incision, and explain at consultation based on your eyelids.
QDo I need double eyelid surgery together with ptosis surgery?
Ptosis Surgery
A line alone on a weak-opening eye may not look as clear as you expect, so the two are often planned together. It is not for everyone; we decide after checking the opening strength at consultation.
QWill my forehead lines improve too?
Ptosis Surgery
As the habit of opening the eyes with the forehead fades, the forehead relaxes and the lines may ease. The degree varies from person to person.
QWill my eyes fail to close after surgery?
Ptosis Surgery
Pulling the muscle more than necessary can leave the eyes closing less fully, so we measure each eye and adjust only as much as needed. The expected range of change is explained at consultation.
QI am worried about medial canthoplasty scars.
Epicanthoplasty
Medial canthoplasty incises the inner corner and closes from the inside, with the incision designed to sit parallel and close to the conjunctiva so the scar is hard to notice. Scarring varies with skin condition and the individual.
QWill the outer corner close up again after lateral canthoplasty?
Epicanthoplasty
Lateral canthoplasty fixes the tip of the outer canthus to the periosteum. Incising the skin alone can let it re-adhere, so fixing is standard; we explain after checking the outer corners at consultation.
QCan it be done together with double eyelid surgery?
Epicanthoplasty
Yes. Canthoplasty is closely related to where the double eyelid line begins, so the two are often planned together. At consultation we look at the balance of the whole eye and decide the order and combination.
QHow long after the first surgery is revision possible?
Revision Eyelid Surgery
It usually takes six months to a year for scars to heal, swelling to go down and the line to settle. Revising earlier can bring side effects back or make the eyes look more unnatural, so we watch the progress and judge at consultation.
QCan I consult even if I had surgery elsewhere?
Revision Eyelid Surgery
Yes. Records of the previous surgery help analyse the cause, and without them we assess your eyelids directly.
QWon't revision make the scar worse?
Revision Eyelid Surgery
Revision usually includes the existing incision line; scar tissue is tidied, tissue preserved and closed with care so the scar does not stand out. Scarring does vary with skin condition and the individual.
QWill my dark circles improve?
Under-Eye Fat Repositioning
Shadow-type dark circles from bulging fat or a hollow tear trough can be expected to improve with repositioning. Dark circles from pigmentation need a different approach, so we confirm the cause at consultation first.
QWill there be a scar?
Under-Eye Fat Repositioning
Both repositioning and removal go through the conjunctiva inside the eye, so no incision is left on the outer skin and there are no sutures to remove.
QHow is it different from lower blepharoplasty?
Under-Eye Fat Repositioning
Under-eye fat repositioning handles only the fat through the conjunctiva when laxity is mild. With marked laxity of skin and muscle, lower blepharoplasty, which incises below the lashes and tidies the skin too, is suitable. We decide together at consultation.
